USNH offers several options for creating forms, event registration, surveys and teacher evaluations.

Microsoft Forms

Microsoft Forms is an application used by USNH for creating online forms that can be used to collect data from others. Microsoft Forms allows users to share and edit results and collaborate with colleagues.

Webforms

Webforms can be built by site owners or form editors to collect information. For help with creating a Drupal or Sites Wordpress webform, please attend a Drupal/Sites Walk-in Session.

Destiny One

Destiny One is the software used by USNH for creating event registration for public and paid events and conferences, including professional development, corporate training, youth programs and summer camps.

Destiny One is a great choice for:

  • Registration for a paid or external event (external events allow sign-ups by the general public)
  • Conference or event registration with multiple sessions or tracks

CRM

Customer relationship management (CRM) software at USNH may include Saleforce, Blackbaud, and Slate.

These platforms can be used for:

  • Lead generation and nurturing
  • Tracking student applicants and inquiries
  • Communication around applications and events
  • Community outreach

Newsletter registration

Constant Contact newsletter sign-ups

Constant Contact is a bulk email-sending service provider that can send mass emails and newsletters to a large group of people at once. USNH community members can contact Constant Contact directly and mention which campus they are on in order to receive a discount for this service.


Qualtrics

Qualtrics is a web-based survey and data analysis tool available to USNH Faculty, Staff and students. Qualtrics is a self-service tool, but there is online training and documentation available online at Qualtrics University, and they offer customer service through their support center.

About forms and collected information

The purpose of a form is to collect necessary information from the university community or the public in order to carry out a specific request, such as a request for information or to sign up for an event. This information is very carefully governed to protect both the privacy of the respondent and the security of the data.

Information protection and classification

All USNH employees have an important role in ensuring the protection of information the university has a duty to protect. Federal laws, state regulations and university system policies require that you protect information about others. This includes information collected about people not affiliated with the university system, on behalf of the university system.

Before you can protect information, you must understand what type of information you are handling. Please review the USNH Classification Policy.

Using webforms to collect information

If you need to collect information classified as sensitive or any information classified as restricted, contact ET&S to discuss potential information collection and storage options that ensure compliance with USNH policy and regulatory requirements.

Special considerations for forms created within a website

Webform information is transmitted via email and stored in the same location as website content. Therefore, it is inadequate protection for any information classified as restricted and most information classified as sensitive.

Collected information should be downloaded and deleted from websites immediately, if possible, and kept in Drupal no longer than 30 days maximum. Data kept in websites longer than 30 days may be automatically deleted by ET&S.

    Responsibility for usage

    Users are responsible for collecting and protecting information in accordance with all USNH policies, federal laws, and state regulations. Failure to protect information can have serious consequences:

    • Harm to individuals whose privacy is breached, such as identity theft
    • Financial and reputational damage to the university system
    • Personal risk if a data breach happens while you are not following policy and law

    For support regarding adding and administering web forms, please contact ET&S.

    Violations

    ET&S Software Development reserves the right to remove at any time any content that it considers in violation of this standard or any other USNH policies.

    If inappropriate or inaccurate content is discovered, ET&S will contact the site owner to discuss the issue. If the site or content owner cannot be contacted or is no longer at USNH, that content will be administered by ET&S and can be removed if deemed inaccurate or inappropriate. When content is obviously unacceptable, ET&S will immediately remove the content prior to contacting the site owner. Unacceptable entries also may cause site access to be suspended.

    Reports regarding inappropriate content may be sent to ET&S Software Development.
