Notice to Voluntary Medical Insurance Participants – 2008 Rate Increase

Employees appointed to a non-status position (excluding students and graduate assistants) for thirty (30) or more days are eligible to participate in the Benefit Value Accident and Sickness Insurance Plan. Employees may enroll after completing thirty (30) days of service. This program offers the opportunity to choose Medical coverage for yourself and your eligible dependents. The plan is affordably priced and your premium payments will be deducted directly from your paycheck.

How to Apply for Coverage:

If you are interested in applying for coverage, please contact your campus Human Resource Office for an application (or click here to print the form).

How to Enroll:

Enrollment is easy:

  1. review the benefits and exclusions,
  2. complete the enrollment form (including beneficiary information),
  3. make a copy for yourself and
  4. return the original to your campus Human Resource Office for processing.

If you decide not to participate as a new-hire, there will be an Open Enrollment period each year.
